This test is done to measure the unbound (free) form of prostate-specific antigen in the blood. It is often interpreted together with total PSA to improve the accuracy of prostate cancer risk assessment. A lower free-to-total PSA ratio may be associated with higher risk of prostate cancer, while higher ratios are more often seen in benign prostate conditions such as BPH.

  • If total PSA levels are in a borderline or intermediate range.
  • If prostate cancer risk needs further evaluation.
  • If distinguishing between BPH and prostate cancer.
  • If a healthcare provider is refining prostate assessment.
  • Fasting: Not required.
  • Avoid ejaculation for 48 hours before testing.
  • Avoid prostate manipulation or cycling before testing if possible.
